BPSM is a Blood Pressure Self Monitoring Program where participants who quality will work with a trained Heart Healthy Ambassador during the 4 month program. Office Hour Appointments will be MONDAY, WEDNESDAY and FRIDAY from 7:30am-8:30am. Appointments begin Monday, August 8th!
HOW THE PROGRAM WORKS
Participants will work with trained Healthy Heart Ambassadors for the four-month program. During this time, participants will be encouraged to:
Self-measure their blood pressure at least two times per month
Attend two personalized consultations per month
Attend 4 nutrition education seminars in a 4 month time period
Participants will record their blood pressure readings using an easy-to-use, self-selected tracking tool. The Y will provide options for participants who need a home blood
pressure monitor.
PROGRAM GOALS
Reduction in blood pressure
Better blood pressure management
Increased awareness of triggers that elevate blood pressure
Enhanced knowledge to develop healthier eating habits
TO QUALIFY, PARTICIPANTS MUST:
Be at least 18 years old
Be diagnosed with high blood pressure
Have not experienced a recent heart attack or other event
Do not suffer from a condition that causes an irregular heart beat
